* The Family Caregiver Support Program (FCSP)
Caregivers of family members are generous and compassionate individuals who give a great deal of themselves to help their loved ones. While there are benefits in being a caregiver, these individuals often find themselves facing physical, emotional and financial difficulties due to taking on the caregiving role. Most find themselves in need of a break to rejuvenate themselves or take care of other obligations.


Caregivers dedicate, on average, 20  hours per week to provide care to their family members. The role of caregiving often takes a high toll on the emotional, physical and financial well-being of the caregiver.

The Family Caregiver Support Program assists two groups of caregivers:

 1)     Family caregivers who care for frail elders age 60 and over and do not have a brain-impairing condition.  (Caregivers of loved ones with brain impairing conditions are served by the Mountain Caregiver Resource Center Caregiver Resource Center (CRC) .

 2)    Those age 60 and over who are the primary caregivers of related children age 18 and under (kinship caregivers).  Related links for kinship caregivers:

There is no charge for the services of the Family Caregiver Support Program. Donations for services are gratefully accepted.

Family Caregiver Support Program services include Information and Assistance, Assessment of caregiver needs, Caregiver Support, Respite Care, Legal Assistance and a variety of Supplemental Services.

Information and Assistance (Information Assistance ) PASSAGES' I & A Department is the agency's central source of information about caregiving and senior issues.  The agency also has an extensive resource lending library for your information needs.

Caregiver Needs Assessment and Care Coordination The FCSP's trained professionals provide comprehensive caregiver assessments through home or office visits and phone follow up.  A plan of care is created based on the assessment and FCSP staff assist in coordinating necessary care and services.

Caregiver Support The FCSP provides caregiver trainings and support groups to help support caregivers in their roles.  We also provide short-term counseling services through licensed professionals to assist clients with problem solving and emotional support.

Respite Care The FCSP provides subsidies for limited in-home care, day care, or institutional care to provide qualified clients with respite.

Legal Assistance FCSP clients are eligible to receive personal consultation with legal services providers for issues relating to their caregiving roles.

Supplemental Services A variety of other services may be provided by the FCSP to qualified clients on a limited basis.  Those services include:                

  • Transportation
  • Assisted Transportation
  • Minor Home Modification
  • Homemaker Services
  • Chore Services
  • Home security and Safety services/devices
  • Assistive Devices
  • Material Aid
  • Translation/Interpretation
  • Visiting Services
