Care Coordination


	We connect people who need help in coordinating their care with the resources they need to live independently. We help delay or prevent the need for institutional care.
Multipurpose Senior Services Program (MSSP) The Multipurpose Senior Services Program (MSSP) is a case management service for Medi-Cal recipients. MSSP's goal is to delay or prevent costly institutionalization of frail elderly living in Butte, Glenn and Tehama Counties. MSSP care management staff arrange and coordinate all needed services for their clients as well as monitor their on-going care and health status.

Linkages Linkages professional staff help coordinate the care of people who, because of age, illness or disability, have difficulty arranging services on their own behalf. Linkages provide services to residents of Butte County who are over the age of 18.
