Multipurpose Senior Services Program (MSSP)
The Multipurpose Senior Services Program (MSSP) is a care management service. MSSP’s goal is to delay or prevent costly institutionalization of frail elderly Butte, Glenn and Tehama County residents. MSSP care management staff arranges and coordinates all needed services for clients as well as monitor their on-going care and health status.
Care Management
MSSP care management teams are comprised of nurses and social workers. Together their roles are:
- Full assessment of each client’s health and other needs
- Arrange for all needed services
- Develop a care plan working with families and other physicians
- Monitoring each client’s well-being through regular home visits and phone calls
Eligibility
MSSP is funded by Medi-Cal and participants must meet each of the following criteria:
- Age 65 or older
- Reside in Butte, Glenn or Tehama County
- Medi-Cal recipient
- Be at risk of skilled nursing facility placement
Services
MSSP arranges for services available in the community and may also help by purchasing specialized services and equipment not otherwise covered by Medi-Cal or Medicare.
Some of those services may include:
- Chore, personal and nursing care
- Counseling
- Transportation
- Medical alert systems
- Respite care for family members
- Medical equipment
- Home modification
