About Connections

A Mental Health Outreach Program

Passages Connections is a mental health outreach program that provides screening, support, and referrals for adults age 60 and older throughout Butte County who are experiencing emotional or mental health concerns, or who would benefit from additional support.

Services are tailored to each individual’s needs and may include home visits when appropriate. In addition to serving older adults, the Connections program provides mental health education, outreach, and training to older adults, professionals, and the broader Butte County community to promote awareness, reduce stigma, and support healthy aging.
